COVID Waiver *
COVID-19 is the disease that is caused by the novel coronavirus (“COVID-19”). It is extremely contagious virus that spreads easily through person-to-person contact. Federal and state authorities recommend social distancing as a means to prevent the spread of the virus. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in programs at Creative Co-Op, LLC (“Creative Co-Op”) facilities could increase the risk of contracting COVID-19. Creative Co-Op in no way warrants that COVID-19 infection will not occur through participation in Creative Co-Op programs or accessing Creative Co-Op facilities.
I hereby acknowledge and agree that participation in Creative Co-Op activities comes with inherent risks.
I acknowledge that Creative Co-Op cannot guarantee that I will not become exposed to or infected with theCOVID-19 during my participation in Creative Co-Op classes or activities.
In consideration of my participation in Creative Co-Op classes or programs, I have read and fully understand and acknowledge the contents of this release and agree that I am voluntarily waiving, releasing, indemnifying, and discharging Creative Co-Op, it’s owners and partners, from any and all liability, damages, and each and every action by participation in any Creative Co-Op class or program including but not limited to exposure or transmission of COVID-19.
My signature below is confirmation that I have read and fully understand and acknowledge the contents of this release and agree that I am voluntarily waiving, releasing, and indemnifying and discharging Creative Co-Op from any claims.
Fitness Waiver
The undersigned understands the risks of group fitness exercise does forever release, acquit, discharge, and covenant to hold harmless Betsy Buccafusca and Creative Co-Op LLC from any and all actions, causes of action, and claims on account of, or in any way growing out of, directly or indirectly, all known and unknown personal injuries or property damage sustained as a result of my enrollment and participation. The undersigned also states that he/she is in good physical condition and knows of no reason why he/she cannot participate in a bootcamp program, STRONG™ Nation class, STRONG™ Nation 30-minute class, Zumba® Fitness class or group exercise program.
The undersigned agrees to observe and obey all posted rules and warnings and further agrees to follow any oral instructions or directions given by the instructor.
In the event of an emergency, I hereby authorize any licensed medical personnel to perform and accepted medical procedure deemed necessary and agree to bear the expense of any such treatment.
